I have 3 live births, all normal spontaneous delivery. And 1 spontaneous abortion/miscarriage in 2002 due to thyroid problem. I was treated with radioactive iodine in 2003.
I gave birth to my youngest in 2004.
My urine always had proteinuria ( ) and hematuria of 30-40/hpf. I had recurrent UTI since 2003. With kidney biopsy, I was diagnosed with SLE with Class II lupus nephritis last Aug 2007, with activity index of 7 and chronicity index of 3. ANA of 1:160 (homogenous)and anti-DS DNA 1:10. From September 2007, I was taking 20mg steroids. It was tapered down to 5mg from December 2007 to present. My urinalysis always show a NEGATIVE ALBUMIN from Sept 2007-present (May 2008). Urine RBC varies from NORMAL to a 30/hpf. My latest urinalysis done last month showed a NORMAL RBC and PUS cells and a negative albumin. Latest serum C3 and Creatinine NORMAL. My 24hr urine protein was 52.18mg/24 hr., creatinine clearance was 157.04 ml/min/1.73sq m. and urine creatinine was 10.29 mg/kg/24 hr. My glucose-FBS was NORMAL. Total cholesterol was 5.21 mmol/L, triglyceride 0.87 mmol/L, VLDL 0.17 mmol/L, LDL 2.73 mmol/L, HDL 2.31 mmol/L, HDL ratio 2.25 mmol/L.
I have no extra-renal manifestation. I never experienced any of the clinical/physical manifestations of SLE - malar rash, arthritis, etc. I am physically active and I don't tire easily. I am a practicing dentist. I am a joyous and spiritually inclined person.
My mom has controlled SLE for more than 15 years now. She is now 66 and is undergoing chemotherapy for her colon cancer, stage 4, diagnosed only last October 2007.
Your test results shows you are in remmision. it will be good to do an ANA profile in case you experience any difficulty.ANA test is positive for more than 95% of SLE cases. ANA negative lupus is very rare , such patients have positive anti ds.DNA or anti Ro . If all the above are negative lupus can be ruled out. . But in many cases these tests get positive about one to two years after onset of symptoms. Hence testing has to be repeated, to confirmly rule out lupus.
class II nephritis have better prognosis compared to III and IV .
But it will be better to have routine tests done regularly. lupus nephritis tends to be an ongoing disease with flares requiring re treatment for many years.
it is so important to keep a n eye on blood pressure , hyperlipidemia, and hyperglycemia.
there is nothing to be worried of. you have rightly given a family history of lupus, that matters.
