I would like to understand how the liver is affected by Lupus and the medication used to treat Lupus. My friend was pulled off all medications, due to bad liver enzeymes or possible liver failure. Why would this happen and how could we handle this. My father died some 30 years ago with SLE and my doctors have told me that I have all elements of the disease but no active disease. I am not sure what that means.

The liver is infrequently affected directly by lupus although on occasion nonspecific liver inflammation appears during lupus flares with mild elevation on blood tests of the so-called liver enzymes, SGOT and SGPT or ALT and AST. Rarely, does this progress to advanced liver disease such as cirrhosis. Additional details regarding lupus and liver disease appears in a previous quesion available on this website. On the other hand, many medications including those commonly used to treat lupus on occasion can have adverse effects on the liver and also lead to elevations of these liver enzymes. In that setting the treating physician needs to monitor the situation closely and determine if the medication needs to be discontinued, possibily reduced in dose or frequency of administration or can be continued with frequent blood testing.
